Pregnancy Passport image.pngPSBC's Pregnancy Passport (PDF) is a booklet to support women in having a healthy pregnancy, tracking their progress, and preparing for their baby. The Pregnancy Passport is available online only. The Pregnancy Passport includes:

  • information relating to your patient's needs and the care they can expect throughout their pregnancy, birth, and after the baby is born;
  • a place to record check-ups and tests; and 
  • a list of resources for more information.

Pregnancy Passport Handout Image.pngPSBC also produces a Pregnancy Passport Handout (PDF) that is available for order. The Pregnancy Passport Handout is published by Perinatal Services BC in partnership with the Ministry of Health and health authorities. The Pregnancy Passport is a companion to Baby's Best Chance.

Indigenous Pregnancy Passport

Our Sacred Journey: Indigenous Pregnancy Passport (PDF) is a culturally appropriate health promotion resource empowering women and families through their sacred journey of pregnancy. The resource incorporates First Nations/Aboriginal traditional beliefs and values as well as clinical best practices.

With the Aboriginal Pregnancy Passport, women can document their sacred traditional journey through pregnancy, birth, and baby’s first few weeks. The passport provides expectant mothers with health information, resources, traditional teachings, growth charts, checklists, and a place to write down goals, thoughts, ideas, and dreams for their babies.
