The Postpartum Nursing Care Pathway identifies the goals and needs of postpartum women and is the foundation for documentation on the BC Postpartum Clinical Care Path (for Vaginal and Cesarean Delivery).
Assessment criteria are organized into three main sections but are not intended to be viewed as separate from one another:
- Physiological Health
- Psychosocial Health
- Changes: Family Strengths and Challenges
Assessments are entered into specific periods, from immediately after birth to seven days postpartum and beyond. These are guidelines to be used to ensure that all assessment criteria have been captured. Once the woman is in her own surroundings, assessments will be performed based on individual nursing judgment in consultation with the mother.
